Hmm well most of my dates involve straight up meeting the girl somewhere. But I did pick up a girl the other day and forgot to open the car door for her lol. I'm sure if it were a fancy night out in the city I would have, but this was a casual coffee shop thing.
But when it comes to the actual date I always hold the door open wherever we go.

You have no idea. It's like a deck of jokers with the exception of 1 ace.

Years ago I was dating a young woman from Novosibirsk, and she would literally stand by ANY door and wait until I opened it for her. That relationship didn't last long.

I do it when I think about it, but don't sweat it if I forget to. I do think about it if we are driving the Corvette and parked near something I don't want her opening the door around as it has some pretty long doors.
In regards to restaurants I walk at a pretty fast pace and am usually at the door first. So out of reflex, I normally hold the door for her to enter first.

I do it because she's my Mami Chula and I love treating her like the queen she is. I pull her chair out for her at restaurants still.

The weird thing with this for me is that I used to do it all the time for my ex. She knew what to expect and made it easy. My current wife is a bit clumsy about it and just opens her own door. I'd like to but it doesn't happen smoothly. I gave up on it. lol

I need to work on my chivalry. I sometimes take off before she's even closed the door. Also never notice when/if she's actually buckled up. Maybe i need to calm down.

Hold the door for my wife and let her walk first always whether it's into a store or building etc. I'll also open the car door for my wife if shes holding something, or were approaching the car and the passenger door is closest to me... I'll open her door first and then walk around just to be a gentleman.
I wouldn't walk past my door around the entire car to open her door, then walk back. It just seems like that is a little much

Depends. Am I driving an e60 M5? Because I've gotta get in and let it warm up for five minutes. If she's fine standing outside the car door for five minutes, then yes, I do open the door for her

If not always, most of the time, and also when no one is watching... Married since 1991 and still doing it... It is how much I respect the mother of our children and my wife... Call me old school, I guess... 58 years young in 2 days...lol
