Thanks for the legit responses so far guys, i know putting the 4th option would lead to some immature people finding their way into this thread, but if u cant have some fun and laugh at some idiots then whats the point anyway?

The reason why I ask the question is b/c i have a feeling not many people grasp the concept of PDF and have acrobat installed and updated to be able to fill out the form electronically. So I was trying to see which option a general majority of people prefer.

how long is it? if its like 3 questions, a simple email would work. does it go into great detail?

2-3 pages of information for clients, i'm just trying to make it an easier process, currently I have them fill it out at the end of our first meeting. I'd rather have them go home and have an e-mail follow-up w/ the attachment to do it at their convenience.

Excellent point with the online form, i hadn't thought about that option. Just send them a link to the page and have them fill it out there. My number one concern is people not having acrobat installed to do the pdf, but I like the interactivity it offers versus a word doc

I don't open attachments or links from people I don't know.
So the best choice for me is plain text in an email. Aside from reputable web pages and surveymonkey.

There's websites online (which other people have already mentioned, but I know for a fact there's more options which are worth checking out) that help you design, send out, and gather/analyze the gathered information (some are at a price I guess, but if it's for your company or something like that vs. for yourself it might be a worthwhile expense). I've had friends at school who have used it for psychology experiments / theses, and I know for a fact people REALLY prefer something that just opens up in their browser, that they can just check off the answers to and save/send without even having anything saved on their computer and without divulging their personal email address in their responses.. I'd go for that!
